About Us

Get your Illinois Medical Card now from a doctor’s clinic that you can trust! Our patients can now use their own medical insurance to be patients at our primary care clinic. After two visits, the physician may recommend you for the Illinois Medical Card.

What is needed?

  1. You must have 1 of the medical conditions listed below as required by the state of Illinois
  2. You will need to provide proof of your identity, Illinois residency, and age (you must be 18 or older to be eligible for certification)
  3. Other important details: fingerprinting (we do this in-house) and 2×2 photo (we’ll take this for you)

I know you want to know the cost:

  1. Doctor visit #1* – We take these medical insurances.
  2. Doctor visit #2*
  3. Illinois Application Fee (Illinois charges you $100, $200, or $250 for 1, 2, or 3 year certifications, respectively or half if you are on disability or are a veteran).
  4. Fingerprinting/background check, application processing, and the photo is $135 – this is done by an outside vendor: Illinois Green Light

*If we do not take your insurance, you qualify for the Self Pay Plan, which is $175 per visit. Normally, a patient-physician relationship takes at least 2 visits, however there are cases that may take longer. This is determined by the physician on a case-by-case basis.

The total cost to get an Illinois Medical Card here in Illinois depends on your insurance, but for a 1 year application not including doctor’s fees, the total cost is: $237.35 ($102.35 for Illinois certification for 1 year and $135 for background check/photo/processing application) or if you are on disability or are a veteran, the total is $186.18 ($51.18 for Illinois 1 year certification). To our knowledge, we are the only office that helps you with your application and takes insurance for your physician visits.

By the way, this is a one-stop-shop for you to get your application sent to the Illinois Department of Public Health. It is also the most cost efficient. WE TAKE MOST COMMERCIAL INSURANCES.
